Description:

We’re hiring a Social Worker to support individuals and families navigating complex challenges — ideal for someone who’s empathetic, resourceful, and committed to creating real impact through human connection. You’ll meet with patients and their families to assess emotional, financial, and social needs during hospitalization. You’ll connect people with community resources such as housing assistance, food programs, mental health services, or job training. You’ll advocate for clients in meetings with schools, healthcare providers, legal systems, or social agencies. You’ll coordinate post-discharge care such as home health, rehab placement, hospice referrals, or transportation services. You’ll be trained to recognize warning signs of depression, anxiety, trauma, or risk behavior and take appropriate action. You’ll work with diverse populations, including youth, seniors, people with disabilities, and those facing poverty, abuse, or trauma. You’ll coordinate with other professionals — from nurses to case managers to counselors — to ensure holistic care. You’ll respond to emergency situations such as code alerts, suicide risk assessments, and family crises with professionalism and clarity.

Basic Qualifications:

  • Assess client needs through interviews, home visits, and collaboration with other professionals.
  • Believe in the power of human dignity, resilience, and meaningful connection.
  • Attend case conferences, court hearings, or interdisciplinary meetings as a client advocate.
  • Respond with empathy and professionalism when handling sensitive or emotional situations.
  • Advocate for vulnerable populations, ensuring access to services and protection of rights.

Preferred:

  • Understands trauma-informed care principles, cultural sensitivity, and inclusive communication practices.
  • Experience working with vulnerable populations such as children, elderly, individuals with disabilities, or those experiencing poverty or trauma.
  • Familiar with coordinating with public agencies, NGOs, or case workers to ensure holistic support.
  • Willing to conduct home visits, school visits, or community outreach depending on program requirements.
  • Willing to participate in trainings, workshops, or ongoing professional development programs.

You’ll provide emergency support when families face eviction, abuse, neglect, or sudden loss, offering stability and immediate options. You’ll lead or co-facilitate parenting workshops, support circles, or family wellness events depending on the program model. You’ll keep up with local service providers, funding changes, and housing availability to make accurate referrals. You’ll receive ongoing supervision, trauma-informed care training, and support from senior social work staff. You’ll work in office settings, schools, hospitals, shelters, or directly in the community, depending on the caseload. No clinical license is required — just emotional maturity, excellent listening skills, and a passion for strengthening families.
