Description:
We’re hiring a Social Worker to join our hospital-based care team — perfect for someone who’s compassionate, organized, and ready to support patients through some of the most difficult times of their lives. You’ll meet with parents, caregivers, and children to assess family dynamics, identify concerns, and create care plans tailored to household needs. You’ll provide bedside counseling, discharge planning, and crisis support for individuals facing serious illness, injury, or life-changing diagnoses. You’ll help patients understand their care options, insurance coverage, and available support services. You’ll provide crisis intervention and emotional support to individuals facing trauma, isolation, or acute instability. You’ll work closely with outreach teams, shelter staff, healthcare providers, and harm reduction specialists to coordinate care. You’ll be trained to work with people who may be resistant to services, coping with mental illness, or living with untreated conditions. You’ll collaborate with teachers, counselors, and juvenile services to ensure at-risk youth receive coordinated care. You’ll document visits, referrals, and outcomes while maintaining confidentiality and trauma-informed practices.
Basic Qualifications:
- Assess client needs through interviews, home visits, and collaboration with other professionals.
- Support clients through crisis situations such as domestic violence, homelessness, or addiction.
- Monitor and evaluate the effectiveness of interventions, adjusting plans as needed.
- Help clients navigate complex systems like welfare, immigration, or mental health care.
- Handle emotional stress in a healthy way and seek supervision or support when needed.
Preferred:
- Familiar with case management, crisis intervention, and referrals to healthcare, housing, education, or legal support programs.
- Open to working in schools, shelters, hospitals, correctional facilities, or nonprofit organizations.
- Willing to participate in trainings, workshops, or ongoing professional development programs.
- Licensed or registered as a social worker (where applicable) and familiar with local laws and ethical guidelines.
- Degree in social work, psychology, sociology, or a related field with relevant field placement or internship experience.
