Description:

We’re hiring a Social Worker to join our hospital-based care team — perfect for someone who’s compassionate, organized, and ready to support patients through some of the most difficult times of their lives. You’ll meet with patients and their families to assess emotional, financial, and social needs during hospitalization. You’ll help assess basic needs such as food, shelter, medical care, addiction treatment, and mental health support. You’ll help patients understand their care options, insurance coverage, and available support services. You’ll document all interactions, progress notes, and service plans with accuracy and confidentiality in mind. You’ll help clients build coping strategies, set realistic goals, and work through crises with patience and structure. You’ll connect youth with mental health professionals, tutoring programs, recreational outlets, or mentorship opportunities. You’ll track progress over time and adjust intervention plans based on evolving needs and family feedback. You’ll document visits, referrals, and outcomes while maintaining confidentiality and trauma-informed practices.

Basic Qualifications:

  • Build trust with clients by being reliable, nonjudgmental, and solution-focused.
  • Attend case conferences, court hearings, or interdisciplinary meetings as a client advocate.
  • Assess client needs through interviews, home visits, and collaboration with other professionals.
  • Recognize signs of abuse, neglect, or exploitation and take appropriate legal steps.
  • Connect individuals and families to community resources like housing, healthcare, or counseling.

Preferred:

  • Able to conduct client assessments, develop care plans, and connect individuals with appropriate resources and support services.
  • Pays close attention to documentation, timely reporting, and maintaining accurate client records.
  • Open to working in schools, shelters, hospitals, correctional facilities, or nonprofit organizations.
  • Background or passion in advocacy, social justice, family services, or mental health support.
  • Thrives in people-focused environments where compassion and structure go hand in hand.
