Description:

We’re hiring a Social Worker to support families in navigating stress, conflict, and life transitions — ideal for someone who believes in healing relationships through empathy, structure, and steady guidance. You’ll work with teens and pre-teens in schools, shelters, or community centers, helping them navigate mental health, peer pressure, and identity issues. You’ll provide bedside counseling, discharge planning, and crisis support for individuals facing serious illness, injury, or life-changing diagnoses. You’ll assist in connecting people to housing programs, income support, rehabilitation, and long-term case management. You’ll coordinate post-discharge care such as home health, rehab placement, hospice referrals, or transportation services. You’ll work closely with outreach teams, shelter staff, healthcare providers, and harm reduction specialists to coordinate care. You’ll connect youth with mental health professionals, tutoring programs, recreational outlets, or mentorship opportunities. You’ll help clients navigate public benefits systems, legal paperwork, and identity documentation processes. You’ll advocate for young clients in meetings with school officials, probation officers, or family support services.

Basic Qualifications:

• Recognize signs of abuse, neglect, or exploitation and take appropriate legal steps.
• Coordinate with schools, hospitals, legal agencies, and non-profits to deliver holistic support.
• Build trust with clients by being reliable, nonjudgmental, and solution-focused.
• Connect individuals and families to community resources like housing, healthcare, or counseling.
• Participate in training, supervision, and continuing education to stay informed on best practices.

Preferred:

• Background or passion in advocacy, social justice, family services, or mental health support.
• Familiar with coordinating with public agencies, NGOs, or case workers to ensure holistic support.
• Organized with managing multiple client caseloads, deadlines, and follow-up appointments.
• Capable of identifying signs of abuse, neglect, or mental health concerns and reporting to appropriate authorities.
• Familiar with case management, crisis intervention, and referrals to healthcare, housing, education, or legal support programs.
