Description:
We’re hiring a Social Worker to support youth and adolescents facing personal, academic, or family-related challenges — perfect for someone who listens without judgment and knows how to build trust with young people. You’ll meet with patients and their families to assess emotional, financial, and social needs during hospitalization. You’ll provide bedside counseling, discharge planning, and crisis support for individuals facing serious illness, injury, or life-changing diagnoses. You’ll help patients understand their care options, insurance coverage, and available support services. You’ll coordinate post-discharge care such as home health, rehab placement, hospice referrals, or transportation services. You’ll be trained in trauma-informed care, domestic safety planning, and culturally sensitive approaches to family systems. You’ll be trained to work with people who may be resistant to services, coping with mental illness, or living with untreated conditions. You’ll track progress over time and adjust intervention plans based on evolving needs and family feedback. You’ll respond to emergency situations such as code alerts, suicide risk assessments, and family crises with professionalism and clarity.
Basic Qualifications:
- Coordinate with schools, hospitals, legal agencies, and non-profits to deliver holistic support.
- Develop and implement individualized care or support plans tailored to each client's situation.
- Participate in training, supervision, and continuing education to stay informed on best practices.
- Document services thoroughly for reporting, auditing, or funding purposes.
- Advocate for vulnerable populations, ensuring access to services and protection of rights.
Preferred:
- Familiar with case management, crisis intervention, and referrals to healthcare, housing, education, or legal support programs.
- Strong interpersonal skills to build trust, defuse conflict, and support clients through emotional or practical challenges.
- Thrives in people-focused environments where compassion and structure go hand in hand.
- Capable of identifying signs of abuse, neglect, or mental health concerns and reporting to appropriate authorities.
- Wants to grow into roles like Licensed Clinical Social Worker (LCSW), Case Manager, or Program Director.
