Description:
We’re hiring a Social Worker to join our hospital-based care team — perfect for someone who’s compassionate, organized, and ready to support patients through some of the most difficult times of their lives. You’ll meet with parents, caregivers, and children to assess family dynamics, identify concerns, and create care plans tailored to household needs. You’ll provide bedside counseling, discharge planning, and crisis support for individuals facing serious illness, injury, or life-changing diagnoses. You’ll help patients understand their care options, insurance coverage, and available support services. You’ll assist in crisis intervention, supporting students experiencing bullying, neglect, abuse, or sudden loss. You’ll assist families in navigating grief, stress, and medical decision-making with calm guidance and empathy. You’ll be trained to work with people who may be resistant to services, coping with mental illness, or living with untreated conditions. You’ll collaborate with teachers, counselors, and juvenile services to ensure at-risk youth receive coordinated care. You’ll help families set healthy boundaries, establish routines, and build conflict-resolution skills.
Basic Qualifications:
- Facilitate support groups or workshops on topics like parenting, coping skills, or life transitions.
- Ensure all actions comply with ethical guidelines, agency policies, and legal standards.
- Adapt quickly to new challenges, whether it’s a policy change or an urgent client crisis.
- Assess client needs through interviews, home visits, and collaboration with other professionals.
- Maintain accurate, confidential case records and update progress notes regularly.
Preferred:
- Familiar with case management, crisis intervention, and referrals to healthcare, housing, education, or legal support programs.
- Experience working with vulnerable populations such as children, elderly, individuals with disabilities, or those experiencing poverty or trauma.
- Willing to help clients navigate systems such as healthcare, housing, education, or employment services.
- Familiar with coordinating with public agencies, NGOs, or case workers to ensure holistic support.
- Comfortable handling sensitive conversations, maintaining confidentiality, and advocating for client needs.
