Description:
We’re hiring a Social Worker to support individuals experiencing homelessness — ideal for someone who’s empathetic, street-smart, and passionate about outreach and connection. You’ll meet with patients and their families to assess emotional, financial, and social needs during hospitalization. You’ll help assess basic needs such as food, shelter, medical care, addiction treatment, and mental health support. You’ll support families facing divorce, custody issues, financial hardship, health crises, or emotional disconnection. You’ll assist in crisis intervention, supporting students experiencing bullying, neglect, abuse, or sudden loss. You’ll help clients build coping strategies, set realistic goals, and work through crises with patience and structure. You’ll assist with home visits, school meetings, and community referrals as part of a whole-family support model. You’ll be trained in HIPAA compliance, cultural sensitivity, and trauma-informed care practices within the hospital setting. You’ll document visits, referrals, and outcomes while maintaining confidentiality and trauma-informed practices.
Basic Qualifications:
- Participate in training, supervision, and continuing education to stay informed on best practices.
- Balance large caseloads while ensuring each client receives consistent attention and care.
- Attend case conferences, court hearings, or interdisciplinary meetings as a client advocate.
- Facilitate support groups or workshops on topics like parenting, coping skills, or life transitions.
- Document services thoroughly for reporting, auditing, or funding purposes.
Preferred:
- Able to maintain appropriate boundaries, manage stress, and practice self-care in emotionally demanding environments.
- Strong interpersonal skills to build trust, defuse conflict, and support clients through emotional or practical challenges.
- Wants to grow into roles like Licensed Clinical Social Worker (LCSW), Case Manager, or Program Director.
- Values empathy, accountability, and empowerment in every client interaction.
- Willing to conduct home visits, school visits, or community outreach depending on program requirements.
