Description:

We’re hiring a Social Worker to join our hospital-based care team — perfect for someone who’s compassionate, organized, and ready to support patients through some of the most difficult times of their lives. You’ll engage directly with unhoused individuals in shelters, encampments, drop-in centers, or on the streets to build trust and offer support. You’ll connect people with community resources such as housing assistance, food programs, mental health services, or job training. You’ll help create safe spaces where young clients feel heard, respected, and empowered to express themselves. You’ll provide short-term counseling or connect families to long-term therapists, legal aid, or parenting programs. You’ll work closely with outreach teams, shelter staff, healthcare providers, and harm reduction specialists to coordinate care. You’ll be trained to work with people who may be resistant to services, coping with mental illness, or living with untreated conditions. You’ll collaborate with teachers, counselors, and juvenile services to ensure at-risk youth receive coordinated care. You’ll advocate for young clients in meetings with school officials, probation officers, or family support services.

Basic Qualifications:

  • De-escalate conflicts and promote communication within families or support networks.
  • Advocate for vulnerable populations, ensuring access to services and protection of rights.
  • Support clients through crisis situations such as domestic violence, homelessness, or addiction.
  • Attend case conferences, court hearings, or interdisciplinary meetings as a client advocate.
  • Work both independently and as part of a multidisciplinary team.

  • Willing to conduct home visits, school visits, or community outreach depending on program requirements.
  • Strong interpersonal skills to build trust, defuse conflict, and support clients through emotional or practical challenges.

  • Willing to participate in trainings, workshops, or ongoing professional development programs.
