Description:
We’re hiring a Social Worker to support individuals experiencing homelessness — ideal for someone who’s empathetic, street-smart, and passionate about outreach and connection. You’ll meet with parents, caregivers, and children to assess family dynamics, identify concerns, and create care plans tailored to household needs. You’ll provide bedside counseling, discharge planning, and crisis support for individuals facing serious illness, injury, or life-changing diagnoses. You’ll help create safe spaces where young clients feel heard, respected, and empowered to express themselves. You’ll coordinate post-discharge care such as home health, rehab placement, hospice referrals, or transportation services. You’ll work closely with outreach teams, shelter staff, healthcare providers, and harm reduction specialists to coordinate care. You’ll connect youth with mental health professionals, tutoring programs, recreational outlets, or mentorship opportunities. You’ll be trained in HIPAA compliance, cultural sensitivity, and trauma-informed care practices within the hospital setting. You’ll document visits, referrals, and outcomes while maintaining confidentiality and trauma-informed practices.
Basic Qualifications:
- Participate in training, supervision, and continuing education to stay informed on best practices.
- Facilitate support groups or workshops on topics like parenting, coping skills, or life transitions.
- Maintain accurate, confidential case records and update progress notes regularly.
- Monitor and evaluate the effectiveness of interventions, adjusting plans as needed.
- Adapt quickly to new challenges, whether it’s a policy change or an urgent client crisis.
Preferred:
- Familiar with case management, crisis intervention, and referrals to healthcare, housing, education, or legal support programs.
- Comfortable facilitating support groups, educational sessions, or family meetings when needed.
- Understands trauma-informed care principles, cultural sensitivity, and inclusive communication practices.
- Able to maintain appropriate boundaries, manage stress, and practice self-care in emotionally demanding environments.
- Open to working in schools, shelters, hospitals, correctional facilities, or nonprofit organizations.

You’ll receive weekly supervision, ongoing case training, and mentorship from senior clinicians or program leads.
