Description:

We’re hiring a Social Worker to join our hospital-based care team — perfect for someone who’s compassionate, organized, and ready to support patients through some of the most difficult times of their lives. You’ll assess client needs through interviews, home visits, and case reviews to develop tailored support plans. You’ll help assess basic needs such as food, shelter, medical care, addiction treatment, and mental health support. You’ll help create safe spaces where young clients feel heard, respected, and empowered to express themselves. You’ll document all interactions, progress notes, and service plans with accuracy and confidentiality in mind. You’ll help clients build coping strategies, set realistic goals, and work through crises with patience and structure. You’ll be trained to work with people who may be resistant to services, coping with mental illness, or living with untreated conditions. You’ll track progress over time and adjust intervention plans based on evolving needs and family feedback. You’ll respond to emergency situations such as code alerts, suicide risk assessments, and family crises with professionalism and clarity.

Basic Qualifications:

  • Handle emotional stress in a healthy way and seek supervision or support when needed.
  • Document services thoroughly for reporting, auditing, or funding purposes.
  • Respond with empathy and professionalism when handling sensitive or emotional situations.
  • Attend case conferences, court hearings, or interdisciplinary meetings as a client advocate.
  • De-escalate conflicts and promote communication within families or support networks.

Preferred:

  • Strong interpersonal skills to build trust, defuse conflict, and support clients through emotional or practical challenges.
  • Able to conduct client assessments, develop care plans, and connect individuals with appropriate resources and support services.
  • Familiar with coordinating with public agencies, NGOs, or case workers to ensure holistic support.
  • Experience working with vulnerable populations such as children, elderly, individuals with disabilities, or those experiencing poverty or trauma.
  • Degree in social work, psychology, sociology, or a related field with relevant field placement or internship experience.
