Description:
We’re hiring a Social Worker to support individuals experiencing homelessness — ideal for someone who’s empathetic, street-smart, and passionate about outreach and connection. You’ll engage directly with unhoused individuals in shelters, encampments, drop-in centers, or on the streets to build trust and offer support. You’ll help mediate communication challenges between family members and promote strategies for respectful, effective dialogue. You’ll support families facing divorce, custody issues, financial hardship, health crises, or emotional disconnection. You’ll coordinate post-discharge care such as home health, rehab placement, hospice referrals, or transportation services. You’ll help clients build coping strategies, set realistic goals, and work through crises with patience and structure. You’ll connect youth with mental health professionals, tutoring programs, recreational outlets, or mentorship opportunities. You’ll coordinate with other professionals — from nurses to case managers to counselors — to ensure holistic care. You’ll help families set healthy boundaries, establish routines, and build conflict-resolution skills.
Basic Qualifications:
- Maintain cultural sensitivity and adapt approaches to diverse backgrounds and values.
- Support clients through crisis situations such as domestic violence, homelessness, or addiction.
- Maintain accurate, confidential case records and update progress notes regularly.
- Help clients navigate complex systems like welfare, immigration, or mental health care.
- Work both independently and as part of a multidisciplinary team.
Preferred:
- Familiar with case management, crisis intervention, and referrals to healthcare, housing, education, or legal support programs.
- Willing to conduct home visits, school visits, or community outreach depending on program requirements.
- Degree in social work, psychology, sociology, or a related field with relevant field placement or internship experience.
- Background or passion in advocacy, social justice, family services, or mental health support.
- Comfortable handling sensitive conversations, maintaining confidentiality, and advocating for client needs.
