Description:
We’re hiring a Social Worker to join our hospital-based care team — perfect for someone who’s compassionate, organized, and ready to support patients through some of the most difficult times of their lives. You’ll work with teens and pre-teens in schools, shelters, or community centers, helping them navigate mental health, peer pressure, and identity issues. You’ll provide bedside counseling, discharge planning, and crisis support for individuals facing serious illness, injury, or life-changing diagnoses. You’ll help patients understand their care options, insurance coverage, and available support services. You’ll assist in crisis intervention, supporting students experiencing bullying, neglect, abuse, or sudden loss. You’ll work closely with outreach teams, shelter staff, healthcare providers, and harm reduction specialists to coordinate care. You’ll be trained to work with people who may be resistant to services, coping with mental illness, or living with untreated conditions. You’ll coordinate with other professionals — from nurses to case managers to counselors — to ensure holistic care. You’ll advocate for young clients in meetings with school officials, probation officers, or family support services.
Basic Qualifications:
- Attend case conferences, court hearings, or interdisciplinary meetings as a client advocate.
- Ensure all actions comply with ethical guidelines, agency policies, and legal standards.
- Monitor and evaluate the effectiveness of interventions, adjusting plans as needed.
- Assess client needs through interviews, home visits, and collaboration with other professionals.
- Respond with empathy and professionalism when handling sensitive or emotional situations.
Preferred:
- Comfortable facilitating support groups, educational sessions, or family meetings when needed.
- Able to work independently or as part of an interdisciplinary team including healthcare providers, educators, and law enforcement.
- Values empathy, accountability, and empowerment in every client interaction.
- Licensed or registered as a social worker (where applicable) and familiar with local laws and ethical guidelines.
- Willing to conduct home visits, school visits, or community outreach depending on program requirements.
